Job Description:/h3:
Job Summary: Identify, provide, and coordinate needed services and benefits to consumers to promote and/or maintain independence within their community setting.
Principal Duties and Responsibilities:
:Reflect Clovernooks mission, vision and core values in daily interactions with consumers, staff, as well as all internal and external constituencies.
:Work cooperatively with other staff, volunteers, and community partners to fulfill Clovernooks mission and vision.
:Identify unmet needs of individuals assigned to caseload; assist consumers, prioritize their needs and recommend options to meet their needs.
:Initiate service referrals, both within the agency and in the community.
:Schedule, coordinate and document Individual Service Plan (ISP) meetings, write and implement case management goals for assigned consumers, and maintain consumer rehabilitation records.
:Document and maintain grammatically correct, necessary records and reports in a timely and accurate manner, consistent with accreditation and quality standards.
:Advocate for benefits which consumers may be eligible to receive, and assist with the process of applying for and maintaining benefits.
:Assist consumers with developing a monthly budget and savings plan; monitor timely payment of rent and other monthly expenses.
:Assist consumers with scheduling medical appointments, monitor medications, assist with obtaining refills for prescriptions.
:Coordinate and provide transportation to consumers when necessary. Monitor the health and safety of the consumer and their home.
:Provide and/or coordinate health and safety training for consumers.
:Act as Plan Coordinator for assigned consumers.
:Attend and participate in staff meetings, ISP meetings, in:service training, as scheduled.
:Identify, promote and facilitate activities which support community inclusion and assist consumers with on:going development of community support systems.
:Utilize Volunteers to assist when appropriate.
:Other duties as assigned.
Job Requirements:
1. Bachelors Degree or equivalent combination of education and experience.
2. Minimum one to three years of experience providing direct support services to individuals.
3. Maintain highly flexible hours to meet the needs of the consumers.
4. Valid drivers license with no more than 5 points on driving record and insured, reliable car available for on the job use.
5. Maintain a high degree of confidentiality on all consumer matters and materials.
6. Maintain the utmost ethical conduct at all times.
7. Post offer, pre:employment drug screen, criminal background check, driving background check.
